HOW HIV RECOGNIZES & CELEBRATES LGBTQ+ COMMUNITIES, NEW RESOURCES ADDRESS HIV IN TRANSGENDER COMMUNITIES... (1703 hits)

June is LGBTQ+ Pride Month—a time to recognize and celebrate LGBTQ+ communities, honor the history of Pride, and commit to working together to ensure continued progress. The origins of Pride Month are deeply rooted in community and political activism. The historic Stonewall uprising that took place the morning of June 28, 1969 served as a galvanizing event—one year later, to commemorate the riots, thousands of marchers took to the New York City streets in the nation’s first gay pride march.

Thanks to decades of tireless advocacy, LGBTQ+ people now hold visible leadership roles in local, state, and federal government; can openly serve in the U.S. Military; and legally marry in all 50 states. In March 2021, the nonpartisan Public Religion Research Institute published a report showing nationwide support for anti-discrimination laws and marriage equality for lesbian, gay, bis*xual, and transgender people reached an all-time high. Progress has also been made when it comes to LGBTQ+ rights and public health, such as the removal of the 22-year travel ban that prohibited all persons with HIV from obtaining tourist visas or permanent residence status, though clearly there is much more to be done—and we, as a society and as public health, must not maintain the systems that support these inequities and propagate injustice.

HIV has been inextricably linked with gay men’s health since the early 1980s, when officials initially termed what would later become known as AIDS, Gay-Related Immune Deficiency (GRID). In these early days of the HIV epidemic, fear of casual transmission meant that many gay men had to fight for visibility and equality not only in the eyes of the law, but also in the eyes of their healthcare providers. Today, LGBTQ+ communities continue to face HIV-related stigma and discrimination, with LGBTQ+ people of color facing even greater challenges. One area of ongoing concern is HIV criminalization laws—legislation specifically passed with the intent of reducing HIV transmission by sanctioning individuals whose behavior potentially exposed people to HIV. Many of these laws criminalize behaviors that do not transmit HIV. Because HIV disproportionately affects Black/African Americans persons, Hispanic/Latino persons, gay and bis*xual men, and transgender persons, HIV criminalization laws are disproportionately applied to persons of color and LGBTQ+ populations.

Transgender women are among the priority populations in the National HIV/AIDS Strategy because of the disproportionate impact of HIV they experience. Three recent resources from the Centers for Disease Control and Prevention (CDC) and the Health Resources and Services Administration (HRSA) are focused on HIV prevention and care in transgender communities.

The first is a CDC issue brief discussing topics specific to HIV and transgender communities. These topics include available data on HIV prevalence and diagnoses, systemic factors that contribute to high rates of HIV among transgender populations, and CDC’s partnership efforts with community-based organizations (CBOs) to intensify HIV prevention and care services in affected populations.

Second is a new CDC MMWR article, Factors Associated with Use of HIV Prevention and Health Care Among Transgender Women — Seven Urban Areas, 2019–2020, that examines structural and health care factors among transgender women that affect their HIV prevention or treatment outcomes. The article notes that among transgender women participating in the survey, experiencing homelessness, poverty, and food insecurity were common and associated with lower likelihood of viral suppression among transgender women with HIV infection. In addition, having a usual source of health care and comfort with a provider were associated with a higher likelihood of viral suppression, HIV testing, and pre-exposure prophylaxis (PrEP) use, all of which play key roles in HIV prevention. The findings underscore that gender-affirming health care is critical to connecting transgender women to HIV prevention and care services, but conclude that access to food, housing, and income are also essential.

The final resource, Transgender Clients Served by the Ryan White HIV/AIDS Program, 2020, is a slide deck from HRSA. It provides aggregate data on over 11,000 transgender clients with HIV, including demographic information, such as housing type and health care coverage, as well as viral suppression rates. Here, HRSA emphasizes that although the proportion of transgender clients whose virus is suppressed has risen over the past decade, transgender clients have a lower a rate of viral suppression than Ryan White HIV/AIDS Program (RWHAP) clients overall. The slides also provide detail on the sub-populations of transgender clients whose viral suppression is lower than the RWHAP average and the average for all transgender clients, including younger clients and clients with unstable or temporary housing.

The mission of the HHS is to enhance the health and well-being of all Americans, by providing for effective health and human services and by fostering sound, sustained advances in the sciences underlying medicine, public health, and social services.

Research Project: We are seeking an individual to collaborate with Dr. Timothy Harrison, Deputy Director for Strategic Initiatives & Senior Policy Advisor at the OIDP and HHS's lead liaison for the National HIV/AIDS Strategy (NHAS), to support efforts related to ending the HIV epidemic in the U.S. initiative (EHE). This research is done in the context of the Syndemics of STIs, Viral Hepatitis, Substance Use Disorders, and HIV.

The individual will document and evaluate the current:

Understanding and/or awareness of EHE and NHAS by federal HIV program staff, HIV service community members and other stakeholders.

Mechanisms OIDP staff uses to educate stakeholders on critical policies that amplify the goals of EHE and NHAS, specifically those impacting the target communities outlined in both initiatives.

Assessment of the effectiveness of communications among federal colleagues related to the implementation and execution of EHE and NHAS, with the inclusion of future recommendations to enhance the public health response to these two vital programs.
Learning Objectives: This opportunity will allow the participant to:

Document programmatic public health program outcomes that will be used to inform multimillion dollar fiscal investments and national policy/guidance.

Attend high level policy meetings to gather information and expand knowledge related to the development of public health policy.

Gain exposure to policy related events and learning opportunities.
Publish evaluation results on HIV.gov and potentially contribute to/write a journal article.

Give presentations on findings at annual conferences.

The appointment is full-time at HHS in the Rockville, Maryland area. Participants do not become employees of HHS, DOE or the program administrator, and there are no employment-related benefits. Proof of health insurance is required for participation in this program. Health insurance can be obtained through ORISE.

The successful applicant(s) will be required to comply with Environmental, Safety and Health (ES&H) requirements of the hosting facility, including but not limited to, COVID-19 requirements (e.g. facial covering, physical distancing, testing, vaccination).
